Funding was awarded by the Centers for Disease Control and Prevention (CDC) to the Iowa Department of Public Health in September 2018 for the grant project "Improving the Health of Americans Through Prevention and Management of Diabetes and Heart Disease and Stroke (1815)". Goals of the Iowa program include increasing screening and testing, enrollment and participation in Diabetes Self-Management Education/Support (DSMES) programs and Diabetes Prevention Programs (DPP); increasing referrals by providers of patients with diabetes and prediabetes to to these programs; increasing awareness of both health conditions; health system procedure and policy changes; increasing employer coverage of DSMES and DPP, and increasing pharmacist involvement in referral, program development, and enhanced medication therapy management (MTM).
