Reinstatement
A licensee whose license has been revoked, suspended, or voluntarily surrendered must apply for and receive reinstatement of the license prior to practicing in this state. Once the license is reinstated, the licensee may apply for active status.
It is the responsibility of the licensee to submit a request to the board seeking reinstatement. The request should contain details that will assist the board in considering the request.
