Applications are reviewed by Board staff typically within five business days. You will receive an email indicating what additional information is required.
Documents may be submitted electronically to the Board office. The Board’s fax number is 515-281-3121. Emailed documents should go to plpublic@idph.iowa.gov.
EXAM INFORMATION
LICENSURE FEES
FEE WAIVER
If you wish to apply for a fee waiver, you must submit documents verifying your household income does not exceed 200% of the federal poverty level. Documents may include:
- Proof of total income for household through other supportive documents
- Copies of the latest federal or state tax returns
- Paystubs
- W-2 / 1099s
- Social Security benefits letter
- Unemployment benefits letter
- Proof of enrollment in a state or federal assistance program
- Medicaid
- WIC
- Supplemental Nutrition Assistance Program
If submitted documentation does not meet the guidelines, you will be required to pay the applicant fee and any associated fees prior to your license being issued.
RENEWAL RULES
You can renew your license up to 60 days before your current license expires. You may begin the renewal process November 2 of even numbered years.

REACTIVATION RULES
INACTIVE STATUS
A licensee who fails to renew the license by the end of the grace period has an inactive license. A licensee who practices in the state of Iowa with an inactive license may be subject to disciplinary action by the board, or criminal sanctions.
REINSTATEMENT PROCESS
A licensee whose license has been revoked, suspended, or voluntarily surrendered must apply for and receive reinstatement of the license prior to practicing in this state.
