What is outreach?
Outreach is one strategy to educate people about the advantages of regular cancer screening, cancer risk factors, and how to access screening services. We must work to raise awareness of the need for cancer screening in tandem with recognition of the Care for Yourself program brand name and services offered.
Making it evidence-based.
The Guide to Community Preventive Services is an evidence-based resource that details strategies that have been found effective in conducting outreach and other services among diverse topics. It is an excellent resource for finding interventions that have been proven to work.

Outreach requirements.
All outreach materials need to include the
- Compliance with the Care for Yourself Brand Identity Guide
- Readable, unedited inclusion of the Care for Yourself logo
- The federal language disclaimer: The following disclaimer must appear on all printed materials used to promote the Care for Yourself program:“This material is supported in part by Cooperative Agreement NU58DP006327-05 funded by the Centers for Disease Control and Prevention. Its contents are solely the responsibility of the authors and do not necessarily represent the official views of the Centers for Disease Control and Prevention or the Department of Health and Human Services.” We understand that the language listed above may be difficult to include on smaller items. If the disclaimer does not fit on the specific outreach material you are implementing, items will be reviewed on a case-by-case basis.
- Proper approval: Outreach materials will not be approved if the proper use of the logos and disclaimer are not included on the requested product(s). All materials must be submitted to Ashley Ezzio (ashley.ezzio@idph.iowa.gov) for approval prior to distribution in the service area. Please allow two weeks for material review.

Reach & Referral.
Reach is the extent to which a program attracts its intended audience. It can be expressed as number of people, organizations, communities, or other value. For our program, reach means the number of people we educated during evidence-based outreach work. Reach helps us track our outreach achievements and progress from year to year.
Referral is the number of people you give enrollment paperwork, call with program information, or send to the website for more information. It will be a portion of the people you talk to at an event. Tracking the number of people we reach and refer to our program can help us measure the success of our outreach efforts. The tracking tools below can help you be successful in documenting reach and referral to your program consultant.
