IMPORTANT NOTICE: Members in a medical institution have full benefits and do not have to pay monthly premiums. Medical institution includes Nursing Facility, Skilled Nursing Facility or Intermediate Care Facilities for Persons with Intellectual Disabilities (ICF/IDs).
Effective September 1, 2018: Every adult member enrolled in the Dental Wellness Plan has an annual benefit maximum of $1,000 per fiscal year (July 1 - June 30). This does not include preventive, diagnostic or emergency dental services, anesthesia in conjunction with approved oral surgery procedures, and fabrication of dentures.
