Iowa Health and Wellness Plan Healthy Behaviors
Members on the Iowa Health and Wellness Plan receive free health services in their first year of eligibility. Members who complete their healthy behavior requirements each year will continue to receive free health services. There are two healthy behavior requirements members need to complete each year:
Healthy behaviors include completion of both:
- Health Risk Assessment
- Wellness Exam OR Dental Exam
Your MCO will send you healthy behaviors reminder letters before the end of your enrollment year:
- Amerigroup - Health Risk Assessment Reminder Letter
- Amerigroup - Wellness Exam Reminder Letter
- Iowa Total Care - Health Risk Screening Reminder Letter
- Iowa Total Care - Wellness Exam/Healthy Behaviors Reminder Letter
