Medicaid Fee-for-Service Provider Network

Dental Provider Networks
The Dental Wellness Plan provides dental coverage for adult Iowa Medicaid members, age 19 and older. For further information on dental carrier provider networks please visit the Dental Wellness Plan Find a Provider webpage.
Dental services are available to Iowa Medicaid members age 18 and younger through the Iowa Medicaid Fee-for-Service (FFS) program. To find a dental provider who accepts Iowa Medicaid FFS coverage, visit the Iowa Department of Human Services Medicaid Provider Search webpage.
Hawki Provider Networks
