Community-Based Neurobehavioral Rehabilitation Services (CNRS)


CNRS is a specialized category of neuro-rehabilitation services provided by a multidisciplinary team of allied health and support staff that have been trained in, and deliver, services individually designed to address cognitive, medical, behavioral, and psychosocial challenges, as well as the physical manifestations of acquired brain injury. Services concurrently work to optimize functioning at personal, family and community levels, by supporting the increase of adaptive behaviors, decrease of maladaptive behaviors and adaptation and accommodation to challenging behaviors to maximize independence.


2021 CNRS Provider Quality Management Self-Assessment 

The CNRS Provider Quality Management Self-Assessment will be required of all enrolled providers offering CNRS regardless of whether those services are currently being provided. All sections of the self-assessment must be completed as requested and submitted as a single document by the deadline indicated each year. Access the CNRS Self-Assessment webpage for more information.
 

Available services under CNRS: 

Residential CNRS 

These services are available to those who currently reside in a three-to-five-bed residential care facility with a specialized license designation issued by the department of inspection and appeals. The intention of the service is to support the member increase of adaptive behaviors, decrease of maladaptive behaviors and adaptation and accommodation to challenging behaviors to support a member to maximize independence. 
 

Intermittent CNRS

These services are available to those who reside in their own home. The intention of the service is to support the member, and the member’s family or caregivers to assist the member to increase adaptive behaviors, decrease maladaptive behaviors, and adapt and accommodate to challenging behaviors to support the member to remain in the member’s own home and community. 
 

Member Eligibility Requirements:

To be eligible to receive CNRS members must meet the requirements outlined in IAC Ch. 78.56(2). These requirements include but are not limited to:
  • The member must have a current brain injury diagnosis with a co-occurring mental health diagnosis
  • The member must exhibit risk factors listed in IAC Ch. 78.56(2)
  • The member must have a completed standardized comprehensive functional neurobehavioral assessment reviewed or completed by a licensed neuropsychologist, neurologist, M.D., or D.O. 90 days prior to admission

Provider Eligibility Requirements: 

To be eligible to offer CNRS providers must meet requirements outlined in IAC Ch. 77.52(2). These requirements include but are not limited to:
  • The agency must be accredited by department-approved, nationally recognized accreditation organization as a specialty brain injury rehabilitation service provider or submit evidence of a completed application within the last 16 months. 
  • The accurate completion of the Annual CNRS Provider Quality Management Self-Assessment form.
  • The agency must demonstrate that they meet all provider standards outlined in IAC Ch. 77.52(2). These standards include, but are not limited to the following: training, governing body, outcome-based requirements, member rights, service documentation and ongoing quality improvement activities.

Quality Improvement Organization (QIO) Specialist

Each CNRS location will be assigned a QIO Specialist to provide technical assistance and provide quality oversight. New providers will be assigned a QIO specialist following the enrollment process.
