Integrated Health Home (for Providers)

An Integrated Health Home (IHH) is a team of professionals, including family and peer support services, working together to provide whole-person, patient-centered, coordinated care for adults with a serious mental illness (SMI) and children with a serious emotional disturbance (SED). This includes individuals currently receiving Targeted Case Management (TCM) and Case Management through Medicaid funded Habilitation. Care coordination is provided for all aspects of the individual's life and for transitions of care the individual may experience. The IHH is required to assist individuals with their paper work and guide them through the application process for benefits for which they qualify. The IHH is required to coordinate all services for an individual, including medical, behavioral, and community services regardless of the funding sources for those services.

Integrated Health Home Standards: 

There are requirements to be met, called Health Home Standards, to be able to participate in the Health Home Program. Minimum requirements listed below:
  • Must be one of the following:
    • Community Mental Health Center
    • Mental Health Service Provider
    • Iowa licensed residential group care setting
    • Iowa licensed Psychiatric Medical Institution for Children (PMIC) facility
    • Nationally accredited by the Council on Accreditation (COA), the Joint Commission, or the Commission on Accreditation of Rehabilitation Facilities (CARF) under the accreditation standards that apply to mental health rehabilitative services. 
  • Provider must be able to provide community-based mental health services to the target population.
  • Providers must meet requirements throughout the state plan amendment.
  • The IHH must have either a Peer Support Specialist or Family Support Specialist.  A Peer Support Specialist is a consumer who is in recovery from a mental illness who has completed 20 hours of training approved by the lead entity and passed a competency exam based on that Peer Support training. 
  • Use some form of a Patient Registry.
  • Electronic Health Record (EHR).
  • Agree to participate in the Iowa Health Information Network.
  • Dedicated care coordinators.
  • Expanded hours for access.
