Pursuant to the authority of Iowa Code Section 249A.4, the Iowa Department of Human Services (DHS) has proposed changes to the state plan under Medicaid State Plan Amendment (SPA) Attachment 3.1-H (Integrated Health Homes: IA-21-003). This SPA is intended to update the Per Member Per Month (PMPM) and fix typos.
The State of Iowa’s Medicaid program currently operates the Integrated Health Home (IHH). The changes to the IHH SPA listed below were developed in partnership and collaboration with the Managed Care Organizations (MCOs) and the Health Homes. The proposed changes are summarized below:

Updated the Health Promotion code from G2058 to 99439 based on CMS Code updates.

Changed the language to align with Team of Health Care Professionals in the Introduction and Executive Summary.

Family Peer support requirements were not updated to align with peer support. This has been fixed.

Updated rates after much stakeholder consideration.
There is no fiscal impact related to the changes proposed in the IHH SPA. The changes in the PMPM amounts are budget neutral.
