The following information is for caregivers of minor and adult patients. Caregivers are designated by a patient's certifying provider to purchase and possess medical cannabis on behalf of the patient.

The minor patient and their parent or guardian will visit with their health care practitioner and ask them to complete a health care practitioner certification form. The form contains the option for a designated caregiver.
Submit your caregiver application. This application includes the patient's completed health care form, your ID, and the application fee of $25.
The Bureau of Cannabis Regulation will review your application and communicate with you via email within 7 days. Once approved, you will receive an approval email that includes your digital card.
Visit a dispensary. Please be sure to have your valid ID and digital card available when visiting the dispensary.
The patient will visit with their health care practitioner and obtain a completed health care practitioner certification form. The form contains the option for a designated caregiver.
The adult patient will submit their application and be given a registration number.
