Thrive Iowa will create a network of navigators to help individuals find immediate support from community organizations, then support them over time helping them develop an individualized plan for self-sufficiency and long-term independence. Across Iowa, countless faith-based and non-profit organizations stand ready to provide goods and support to Iowans. Yet too often, those who need immediate assistance don’t know where to turn for resources. Thrive Iowa will tap into these resources and provide Iowans with the support and relationships to help them succeed.
Thrive Iowa Model
Iowa is implementing Thrive Iowa utilizing the Restore Hope model created by Arkansas-based non-profit, Restore Hope.
How it works:
Thrive navigators are experts in their communities and work directly with families to connect them to services and resources.
HopeHubis a collaborative case management system that connects organizations to create a care team and make referrals from one provider to another, ensuring that services are received and documented. Members of the care team can provide updates securely to ensure constant communication and coordinated services.
The system also serves as the mechanism to track participant outcomes in 13 domains, including: housing, recovery, employment, transportation, education, mental health, physical health, safety, dental, financial stability, food, child care and legal.
Community Alliances are formed in each community implementing Thrive Iowa. Each alliance meets monthly. The Thrive Coordinator brings together partners from many organizations and entities from different sectors to integrate their agenda into an inclusive, common effort to help families in their communities. A commonly used tagline of the initiative is, “For the community, by the community.”

The collective impact model requires a common agenda, shared measurements, mutually reinforcing activities, continuous communication and backbone support. Each community implementing Thrive Iowa will be engaging partners to collectively move families from crisis to career. Partners include non-profits, for-profits, faith-based organizations, government entities, and volunteers. All interested members of a community are welcome to become part of the community alliance. Resource navigation is only successful in improving family outcomes if there are high quality services, concrete goods and resources to refer to and rely on. Volunteers can play an important role in helping families in their communities reach their goals. Connecting with the Thrive coordinator is the best way to get involved.
