Provider Cancellation of a Patient or Caregiver Certification Form
This form is used by a certifying health care practitioner to request cancellation of a patient's or caregiver's medical cannabis certification. Upon receipt, HHS will rescind the certification, with cancellation becoming effective 30 days after the provider's signature to allow time for the patient or caregiver to use or properly dispose of any remaining medical cannabis products.
