Iowa offers a state Family Planning Program (FPP) which provides health coverage to individuals ages 12-54 with a limited income.
The Family Planning Program is a confidential service.

The FPP helps Iowans plan for growing their family by providing resources and education to plan for support, financial independence, and more. This includes:
- Increase the spacing between births
- Improve future birth outcomes
- Ensure Iowans have access to essential family planning and sexual & reproductive health services
Covered Services
- Birth Control Exams
- Birth Control Counseling
- Limited Testing and Treatment for Sexually Transmitted Diseases (STDs)
- Pelvic Exams
- Pap Tests
- Pregnancy Tests
- Birth Control Supplies
- Voluntary Sterilization
- Emergency Contraception
- Ultrasounds (if medically necessary and related to birth control services)
- Yeast Infection Treatment
Where to Get Care
Any FPP provider enrolled with Iowa Medicaid can provide family planning services to FPP members. Providers include:
- FPP Family Planning Clinics
- Medical Clinics
- Federally Qualified Health Centers (FQHCs)
- Rural Health Clinics (RHCs)
The provider search tool will take you to the Iowa Medicaid Enterprise (IME) Provider Search Portal. Select 'FPP Provider' in the 'Type' dropdown. You can change the 'Specialty,' 'County' or 'Metro' to expand or narrow your search. Provider lists are subject to change and are updated daily.

To be eligible for this benefit program, you must meet be between ages 12 and 54, and:
- Be an Iowa resident
- Be a U.S. citizen or qualified immigrant
- Meet income limits (includes moderate incomes)
Women whose pregnancies and deliveries were covered by Medicaid will have family planning services covered for an additional 12 months without having their eligibility re-determined.

Note: The income does not apply if a woman qualifies because she was receiving Medicaid when her pregnancy ended.
The gross income (before taxes and other deductions) received by any person included in the household size must be counted. Income received by a parent for a teen (e.g. child support, social security benefits) is not counted for the teen’s eligibility unless the parent makes it available to the teen.

You can apply for the FPP at a designated Title X family planning clinic and at the local HHS office in your area.
- Remember that your income, citizenship, and identity must be verified.
- Services will be available immediately; however, it may take up to 30 days for an eligibility determination to be made.
Documents You'll Need
To apply for FPP, you'll need to provide:
- Proof of citizenship (such as birth certificate or passport)
- Non-U.S. citizens need to bring immigration documents
- Proof of Identity (such as government issued driver’s license or ID or student ID)
- Social Security Number
- Proof of income from last 30 days:
- Pay stubs
- Statement from employer
- Child support received (payment record or court order)
- Other requirements may apply
Even if you don’t have these documents you can still submit an application.
