Disability Access Point Plans

  • Disability Services Town Halls (see below): Use presentations and recordings to learn more information that is focused on members and providers (April 2025 – September 2025)
  • Continuity of Care: Iowa HHS, DAPs and providers work together to ensure that individuals continue to get the care and services that they need. DAPs will connect with members, so each individual is aware of their specific plan and transition.
  • Aging and Disability Resource Center (ADRC): The Aging and Disability Resource Center (ADRC) network is made up of organizations with the purpose of helping to make it easier for older adults and people with disabilities in Iowa to find the services and support they need.
  • Iowa Developmental Disabilities Council (DD Council): Iowa’s DD Council was created to advocate for Iowans with disabilities so they can live and be a part of their community and help support the Developmental Disabilities Assistance and Bill of Rights Act. The Council includes people with disabilities, family members, state government leaders and other groups that advocate for people with disabilities.
  • Conner Training Connection: The Conner Training Connection’s Mission is to support Iowans with disabilities to live as fully participating members of their communities. This is done through the provision of training resources and technical assistance to individuals, family members, service providers and other disability advocates.
  • Center for Disabilities and Development: The Center for Disabilities and Development at University of Iowa Stead Family Children’s Hospital supports Iowans with disabilities, their families and providers. This is done by providing evidence-based clinical services, statewide community programs, interdisciplinary education and training, and state-of-the-art research.
