What is the HOME Project?

Through the HOME project, Iowa HHS is working to improve behavioral health, disability, and aging services so Iowans can receive support in their own communities and remain connected to family and friends.

The project began in 2022 when Iowa HHS partnered with Mathematica to evaluate Iowa's home and community-based services (HCBS) system. Feedback from Iowans helped identify three key areas for improvement:

  • Case management
  • Assessment processes
  • HCBS waivers

These focus areas are helping guide efforts to strengthen services and better meet the needs of Iowans.

This graphic (73.58 KB) .pdf shows the progress Iowa has made since 2022 to evaluate and begin making improvements to programs and services. It also shows the changes that are coming up in the next few years to build a coordinated system with better access to services.
