How to Apply for Hawki

Below are some of the eligibility requirements to determine if your child qualifies for the Hawki program:

  • A resident of the state of Iowa
  • Under 19 years of age
  • Have no other health insurance
  • A citizen of the United States or a qualified alien
  • Be in a family that meets the Hawki income limits (Check your family's income in the Hawki Income Guidelines table to see if your children qualify for Hawki)
  • Not a dependent of a State of Iowa employee
  • Not currently covered under the Medicaid program

Fill out an application, or call Hawki, if you think your children may qualify. 

Convenient ways to file an application for the Hawki program:

  1. Online by using the DHS Services Portal.
  2. Print and fill out the Application (Form 470-5170). (Spanish Form 470-5170S). Mail the application to:

  1. Call Hawki Member Services at 1-800-257-8563 and ask for an application to be mailed to you.
