For tax year 2025, Iowa HHS will not automatically mail 1095-B forms. You will only receive this form if you request a copy.

When the Affordable Care Act (ACA) started in 2014, taxpayers had to pay a penalty if they didn’t have health coverage. The 1095-B form was used to show the IRS that you had coverage. Starting in 2019, these penalties ended, so the form is no longer needed to file your taxes.

How to request your 1095-B form:

• Email a request to: dhscontactcenter@hhs.iowa.gov
• Call and request:

Please include the following information in your request:

• Your name
• Mailing address
• DOB

Once we receive your request, a copy of your 1095-B form will be mailed to the address you provide within 30 days.

Call the Income Maintenance Customer Service Center to report changes such as:

• Address
• Birth of a Child
• Employment Starts or Ends
• Other income or household member changes
