- Burial-Transit Permit
- The burial-transit permit can be printed on plain bond paper. If using for out-of-state or out-of-country, we suggest that funeral directors print it on the special registration paper that has the VR seal on the back. Print the form on the 'white' side of the paper.
- Supplies of the special registration paper can be obtained by faxing a statement on the funeral home's official business letterhead to 515-281-0479 ATTN: SUPPLY CLERK. Or, if only needing a few sheets, ask your local County Recorder for a small supply. The "special registration paper" is the same paper that was used for the death certificate form prior to the EDRS.

- The Certificate of Fetal Death must be printed on the special registration paper. Some hospitals maintain a few pre-printed forms so that they may initiate the record -- so communication with them is important. If the funeral home is printing the form, be sure to print the main form on the 'white' side of the paper and side two with the statistical information on the blue side with the VR seal landing in the lower left corner so it doesn't obscure any text.

- Disinterment Permit Application
- The Disinterment Permit must be printed on the special registration paper. Be sure to print the main form on the 'white' side of the paper and side two with the instructions on the blue side with the VR seal landing in the lower left corner so it doesn't obscure any text.

A certified copy of the court order must also be presented to the state vital records office in addition to completed forms and affidavits. Follow the instructions detailed in the Administrative Code (aka Rules).
Forms must be printed on the special registration paper, with side one of the form printed on the white side of the paper. The special registration paper can be obtained from the state vital records office at no charge.
Contact the Bureau of Health Statistics and Vital Records if you are assisting with a Delayed Certificate of Death -- that is, for a death event that occurred more than one year ago.
